Fatal Tiger Attack at Wildlife Park Triggers Dual Investigations
Ambulance service probes potential data breach following Doncaster incident; council and police launch inquiries.

William Chapman died on Sunday after a tiger attack at Yorkshire Wildlife Park in Doncaster.
The Yorkshire Ambulance Service Trust investigates two of its staff members. They may have accessed or attempted to access Mr. Chapman's confidential medical information without legitimate reason.
A spokesperson for the trust confirmed two instances where staff accessed or attempted to access incident or medical information following this incident. They had no legitimate reason to do so.
The trust reported the matter to the Information Commissioner's Office (ICO). Mr. Chapman's family received notification of the data access concerns.
This incident follows a similar situation. Ten staff members were recently removed from duty at East Suffolk and North Essex NHS Foundation Trust for suspected record snooping.
